West Virginia University Benefits Open Enrollment 2007-2008

5/2/07 update
PEIA EXTENDING ENROLLMENT UNTIL MAY 4

The WVU Benefits Administration office has just received information that PEIA has announced that they are extending the deadline for benefits open enrollment until May 4, 2007. This includes on-line enrollments at www.wvpeia.com as well as paper (hard copy) transfer form and tobacco affidavits.

Forms may be found at http://www.hr.wvu.edu/benefits/2007-2008openenrollment/index.cfm as well as at the WVU Human Resources Office.

Mountaineer Flexible Benefits (FBMC) open enrollment ended April 30, 2007. FBMC is not extending their deadline. No forms will be accepted for open enrollment purposes for FBMC.

PEIA forms may be submitted to the WVU HR Benefits office, One Waterfront Place, 2nd Floor, PO Box 6640, Morgantown, WV 26506-6640, or may be placed in the WVU HR 24 hour drop off box in front of One Waterfront Place. The extended deadline is May 4, 2007.

• Authorization to Conduct Online Transactions form
• 2007-2008 Replacement Tobacco Affidavit and Open Enrollment Transfer form - only for employees making changes to their health coverage in addition to providing their tobacco status which may allow them to take advantage of the tobacco free discount
• 2007-2008 Tobacco Affidavit ONLY - for employees making no changes to their health coverage but who wish to take advantage of the tobacco free discount
